To remain compliant with HIPAA and privacy laws, Mutual Health Services administers the provider portal setup. If you have questions before you receive set-up approval, please contact our Customer Care department at 800.367.3762.

Online Access Instructions

To gain access to our provider portal, please submit the following:

• Copy of a signed W9 Form
• Request for online access, which includes name(s) of authorized user (first, last and middle initials) and email address(es), printed on company letterhead and signed by an authorized official
• Email address

To reduce the possibility of unauthorized access, you must notify Mutual Health Services when an authorized user is terminated to ensure their access deactivated.
